Once you have decided to begin a training/wellness program with me, please print out, complete, and bring the following forms with you (or email them to me).
Physical Activity Readiness Questionnaire (PAR-Q)
This form will indicate if you are physically ready to participate in a physical activity program
This form gives me permission to work with you
All information collected is confidential and for the purpose of creating an individualized physical activity/wellness program for you
This form will help to indicate any contraindications to exercises and provide health considerations in designing an appropriate fitness program
If the results of your PAR-Q Form indicate that you should seek medical permission in order to participate in a physical activity program, print and complete form before our first training session
7- Day Nutrition Challenge Scorecard
Each day you will focus on a new nutrition challenge, while completing the challenge from the previous day as well. Good Luck!
